Please complete and send the required documents to TPPC for processing. Scanning and uploading the forms to MSS is the most secure and efficient method of submitting forms. Forms can also be emailed, faxed, hand delivered or mailed. Note completed online beneficiary forms must be signed and physically sent to TPPC.
